OHSU

Students attending clinical at Doernbecher (OHSU) must complete the following forms and training modules prior to the first day of clinical. Students will be informed about Inpatient Epic training via email.

To obtain your OHSU ID card take all certificates from the online modules listed below, signed Background History Certification letter (get this from Emily Heuman in BC 315) and the Student Access & ID card form (contact Melanie Blehm at OHSU to arrange a time to obtain the Access &ID card form blehm@ohsu.edu) to OHSU Parking. This needs to be completed before clinical starts.

The cost to purchase an OHSU ID card is $25.00 at OHSU parking and transportation. Students are required to have an OHSU ID card while attending clinical at this facility. The University of Portland School of Nursing will reimburse students for ID card purchase ONLY. Please bring the original receipt from OHSU Parking and Transportation to Kelly Margheim in BC 315. Make sure that the receipt has the company name listed at the top.
