School of Nursing: Professional Practice Requirements for Spring 2020

Listed below are all the required forms, items to be purchased, and processes to complete in preparation for your Professional Practice courses in the University of Portland School of Nursing (SON) program. All clinical requirements must be submitted into a clinical requirement system on or before February 3rd, 2020. An invitation email to join the clinical requirement system will be sent in December to all students registered to take NRS 206 in the Spring. Please complete all the requirements listed below. ITEMS TO SUBMIT INTO THE CLINICAL REQUIREMENT SYSTEM BY THE DEADLINE

Clinical Requirement Checklist

The SON must verify that all nursing students have up-to-date immunizations and proof of active immunity. All upper division students must complete these immunizations in order to attend clinical experiences. The School of Nursing Immunization Guidelines provides details about how to meet immunization requirements. All records will be uploaded into the clinical requirement system. It is your responsibility to update any expiring clinical requirements annually and to upload documentation of this while in the nursing program.  

Best practice: Have items 1~5 & your American Heart Association BLS CPR card ready to upload by Jan. 5, 2020. Invitations will be sent in December to set up your online account.

  1. Tetanus/Diphtheria/Pertussis (Tdap, DTaP) - Provide documentation/proof of full Tdap within the last 10 years. Td-only boosters are not accepted. 
  2. MMR (Measles/Mumps/Rubella) - Provide documentation/proof of either two vaccinations (>28 days apart) or three titers that show immunity to all three diseases.
  3. Hepatitis B (HBV) Series - By the deadline, student must provide one of the following options:
    1. Proof of 3 childhood Hep B shots + a positive/reactive Hep B titer result (immunity achieved)
    2. Proof of 3 childhood Hep B shots + a negative/nonreactive/equivocal Hep B titer result + a booster Hep B shot after the titer.
      • See Hep B (HBV) protocols flowchart for stepwise guidance. Documentation is required at each step of the flowchart. Students who enter the program while not immune must continue along this flowchart until immunity is documented or they are found to be a non-responder.
  4. Tuberculosis (TB) - Either two skin tests or one blood test meet this requirement. See TB Testing Guide for details. 
    1. Skin testing (PPD tests): two tests are required
      • The first TB skin test must have been completed between Sept. 1, 2019 and Jan. 20, 2020
      • The second skin test must be completed between Dec. 10, 2019 and Jan. 27, 2020.
      • Two appointments 48-72 hours apart for each test, 1 week minimum between first and second test (4 appointments total!)
    2. Blood testing:
      • One QuantiFERON TB Gold test must be completed between Dec. 10, 2019 and Jan. 27, 2020.
    3. *If you have a history of positive TB, submit:
      • The positive TB test indicating history of infection
      • A clear chest x-ray from within the past 5 years
      • A review of symptoms form or note from a provider indicating no symptoms of active TB infection
  5. Varicella (Chickenpox) - Provide documentation of either two vaccinations (>28 days apart) or a titer to show immunity
  6. Influenza vaccine - To be completed annually during flu season starting in October.

 

American Heart Association BLS CPR Certification

All students are required to have current Cardiopulmonary Resuscitation (CPR) for BLS Provider (Basic Life Support of Healthcare Providers)This certification includes Automatic External Defibrillator (AED), infant, child and adult CPR. Certification must be obtained through the American Heart Association. CPR certification through other institutions will not be accepted. Training must include psychomotor and written testing. Certification must be kept current through graduation. 

Keep in mind that if an instructor does not provide CPR cards at the end of the CPR class you must ask for a letter. This letter will be a temporary placeholder until the official card is available.

Criminal History Background Check

Completed through online system. Once ordered, no action is needed. Instructions will be sent in December. If you are studying abroad in the spring semester, please inform Nozomi Birkett at birkett@up.edu. They will send you an earlier timeline via email.

 

Drug Testing

Bloodborne Pathogen, OSHA, HIPAA training

Information will be sent at a later date from your NRS 206/207 instructors. These will be completed in class.
