NCLEX Testing and RN Licensure

The following information is provided to help you navigate the process for applying to take the NCLEX-RN exam and applying for licensure in the state of Oregon. There are two different processes that should be completed simultaneously 4-6 weeks before graduation: (1) OSBN application packet and (2) registering for the NCLEX-RN exam with Pearson VUE (testing agency). To avoid delay in processing your application you must use the same name on every document you submit to the Oregon State Board of Nursing ("OSBN") and Pearson VUE. Since you will be required to show identification at the testing site, you should use the name printed on your legal identification.

Two applications that must be completed before you take NCLEX-RN in Oregon.

1. RN Licensure

  • You can only apply for initial licensure in one state. After you have passed the NCLEX and have received your initial license, you can apply for licensure by endorsement in additional states.
  • OSBN Licensure by Examination. Each student applying for licensure in Oregon is responsible for completing and mailing the OSBN licensure packet 4-6 weeks before graduation. This will give the OSBN plenty of time to process your application. Students must download and complete the Licensure by Examination Application forms. Students are responsible for mailing to OSBN the following items:

    o Completed Licensure by Examination Application;
    o Check or money order in the amount of $169.00
  • Part of the OSBN process involves obtaining the dean's signature on the Examination Picture Identification form. To obtain the dean's signature, please bring your completed Examination Picture Identification form to Anaraquel Aguilera, in BC 301 by March 30, 2017 for verification. Applicants must attach a passport photo using glue stick or double-sided tape. No staples, please. Passport photo must be taken within the last 6 months. You only need to complete the top portion of the form. The School of Nursing will then forward your Examination Picture Identification form directly to the OSBN for processing.
  • You must also submit the OSBN Licensure Transcription Request form together with the Registrar's Office Transcript Request form to the Registrar's Office in Waldschmidt Hall 100 before graduation. Please note: On the Registrar's Office Transcript Request form, select the box to the right that states: "Send after grades have been posted" and insert "Spring 2016." The OSBN will not issue your license if they do not have a copy of your official transcripts on file.
  • For students with testing accommodations: In accordance with the Americans with Disabilities Act (ADA) testing accommodations such as extra testing time or a recorder may be given to examination candidates with documented disabilities who demonstrate need. For more information regarding eligibility and approval, refer to the current NCLEX Candidate Bulletin posted on the NCSBN website.
  • OSBN requires each applicant to be fingerprinted. New this year, the OSBN is requiring all applicants to use electronic finger printing service, Fieldprint Inc. Fingerprinting is required for the national fingerprinting-based criminal background check. All information regarding fingerprinting will be sent to you via email shortly after you submit your application from OSBN. You will pay with a credit or debit card a total amount of $64.50 for this service.

  • Applying for RN licensure outside of Oregon. For those students seeking licensure in a state other than Oregon, you must contact that state's board of nursing for their application materials. Most states have their licensure by examination application online. If the state you are seeking licensure in requires a program verification or certificate of completion to be completed by the dean after graduation, please turn those forms into Anaraquel Aguilera, BC 301, by March 30, 2017.
2. NCLEX Examination Application.
  • Go to the Pearson VUE website and follow the registration instructions.
  • University of Portland program code is: US80505100.
  • Registration fee is $200.00, payable using a MasterCard, Visa or American Express debit/credit card.
  • After graduation, provided you have registered with Pearson VUE and the OSBN application is completed and processed you will receive an Authorization to Test (ATT) via email from Pearson VUE. Only then can you schedule a testing appointment on the Pearson VUE website. PLEASE NOTE: The OSBN requires up to 10 days after the University of Portland has submitted the Candidate List to release your name to Pearson VUE.
*** Pain and HIV/AIDS Curriculum Content Reports for Licensure
  • BSN graduates may need curriculum reports for pain content and skills and HIV/AIDS content and skills when applying for licensure or renewing their license. Graduates preparing to apply for licensure in Washington are required to answer a question regarding HIV/AIDS education on their application form. The 2016 HIV/AIDS Content and Skill information will be posted here around March 2016. Graduates applying for licensure in a state other than Washington, may be asked to verify that they have received Pain and HIV/AIDS education when they renew their license. The 2016 Pain Content and Skills information will be posted around March 2016. Graduates should keep these documents as they may be needed when applying for RN licensure or license renewal.
