Legacy Training

Students attending clinical at Legacy (Randall Children's, Good Sam, Emanuel, Meridian Park and Salmon Creek) must complete the required items listed below.

Required Training and Paperwork
Legacy E+Training for Epic- usernames and passwords are provided via email from School of Nursing.
Legacy Nondisclosure Agreement - print, fill out and return to School of Nursing. Please ask anyone to sign the witness signature portion of this document.
Legacy Stat Strip Glucometer Training Module - sign into the guest book and for the user ID use the number zero. Provide certificate to School of Nursing.
Criminal History Background Check - results are sent directly to the School of Nursing
Legacy Health System ID Badge form- Fill out and take this form to security for your ID at any Legacy Hospital.
Legacy Policy and Emergency Codes - just needs to be reviewed.
 Legacy HIPAA Privacy and Security Training- just needs to be reviewed.
 Legacy HITECH Breach- just needs to be reviewed.
