DNP Clinical Requirements
DNP STUDENTS ONLY
Listed below are all the required forms, items to be purchased and processes you must complete. All clinical requirements must be submitted into a clinical requirement system called ACEMAPP by communicated deadlines. A separated email will be sent out invited you to join ACEMAPP. Make sure you complete the registration process and all the requirements listed below. Any questions regarding these forms can be directed to Michelle Bickley at bickley@up.edu.
CLINICAL REQUIREMENTS
Items to be completed and uploaded into ACEMAPP before the deadline:
1) Immunization dates and documentation/proof
2) Copy of BLS Health Care Provider CPR certification under the American Heart Association guidelines
Other items to be completed before deadline:
1) Criminal Background Check - use link listed below
2) Orientation Training - to be completed in ACEMAPP
3) NPI # (National Provider Identifier)- fill out online form
4) Health Insurance Required - proof of current coverage must be maintained in ACEMAPP
5) Drug Testing - see email for information to complete this requirement
SCHOOL OF NURSING IMMUNIZATIONS
The SON must verify that all nursing students have up-to-date immunizations. All entering students must complete these immunizations in order to attend clinical experiences. The School of Nursing Immunization Guidelines provides details about how to meet immunization requirements. All records will be uploaded into ACEMAPP. It is your responsibility to upload new documents when they expire while in the nursing program.
SON Immunization Guidelines
- Diphtheria/Tetanus/Pertussis - Provide documentation/ proof within the last 10 years.
- MMR (mumps, Measles, Rubella) - Provide documentation/proof of two vaccinations or a titer.
- Hepatitis B Series - Provide documentation/ proof of three vaccinations AND a blood test titer verifying immunity to the disease.
- Tuberculosis (two skin tests (four office visits) or TB blood test)
The first TB skin must have been within the last twelve months. The second skin test must be no more than three months old. If you have not had a TB skin test within the last twelve months, please get two TB skin test (four office visits). OR, one blood test (QuantiFERON TB Gold test). This will be done instead of the two TB skin test listed above. Provide documentation/ proof.
- Varicella - Provide documentation/ proof of a titer to show Immunity (had virus) or two vaccinations.
- Influenza - documentation of current season's flu vaccine available October/November.
CPR CERTIFICATION
All students are required to have current Cardiopulmonary Resuscitation (CPR) for Basic Life Support Health Care Providers that includes the use of Automatic External Defibrillator throughout their clinical experiences. All students must be certified through the American Heart Association guidelines. Certifications from any other association will not be accepted. You will not be allowed to attend clinical if your CPR is not current or the correct type. Certification must be kept current through graduation.

Criminal History Background Check

The Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) mandated the adoption of standard unique identifiers for health care providers and health plans. The purpose of these provisions is to improve the efficiency and effectiveness of the electronic transmission of health information. The Centers for Medicare & Medicaid Services (CMS) has developed the National Plan and Provider Enumeration System (NPPES) to assign these unique identifiers.

1) White lab coat
· Must be all white
· Must be long sleeved
· Must have pockets
· Must NOT be longer than thigh length
· Do NOT add any embroidery
2) University of Portland School of Nursing Patch
· Purchase one patch from the University of Portland bookstore to be attached to white lab coat. Iron onto left sleeve, 3" from shoulder seam and centered left to right, and secure with same color thread as border.
